Job Title: Licensed Practical Nurse
Job Location: Bergen Gardens (1475 Molson)
Reporting Manager: Community Health Director

Job Summary:

Reporting to the Community Health Director, the Licensed Practical Nurse will be responsible for providing nursing-related care in accordance with policies and procedures; this includes making regular rounds, administering medications, and performing professional procedures and treatment services as required within the scope of practice. The Licensed Practical Nurse will also coordinate with other care partners’ services required to meet the elder’s needs.

WHAT YOU’LL DO:

Communicate with elders’ physicians, using the SBARR, as necessary in planning and providing safe elder care.
Coordinate with other care partners services required to meet my elder’s needs including infection control, foot care, pharmacy, laboratory services, massage therapy, the dietitian, and rehabilitative medical services. Schedule appointments and complete referrals as required.
Ensure that all components of the elder’s health assessment and care plan are completed/reviewed when moving in, quarterly and as needed, evaluating the plan of care in relation to expected outcomes.
If necessary, modify the plan of care to achieve identified expected outcomes or redefine new outcomes.
Communicate when maintaining the elder’s health records and perform charting duties. Ensure that all records are informative, accurate, and current.
Assist on an elder’s move-in day as requested by the Community Health Director. This includes completing assessments, completing the “Move-In Checklist”, and orientating our new elder and their family and/or friends to Bergen Gardens.
Receiving Physician orders and following the process for medication changes accurately.
Participate in the medication reconciliation process in the interest in elder safety.
Maintain and monitor records of narcotics/controlled medications and emergency medications used during the shift.
Communicate directly with elders and/or their family in relation to changes in condition, care plan, safety issues or occurrences that the elder has been involved in.
Perform diagnostic techniques, including venipuncture and specimen collection (e.g., blood glucose monitoring, urine collection, swabs, cultures, and other laboratory tests ordered by the Physician or Nurse Practitioner) within the scope of practice.
Evaluate the results received and ensure that the Physician or Nurse Practitioner has been notified of the results in a timely manner.
Other duties as assigned.

Qualifications

What You Need to Be Successful:

Currently registered and in good standing with the College of Licensed Practical Nurses of Manitoba.
Practical understanding of how a long-term care facility normally operates.
At least 3 years of experience in Health Care and/or am knowledgeable about the principles and philosophy of long-term care with leadership abilities/initiative to effectively co-ordinate all activities and services within the neighborhood and take on the Charge Nurse role as the need arises.
Ability to communicate with well-developed written and verbal communication skills.
Ability to fluently read, write and speak English. The ability to speak a second language would be considered an asset.
Ability to demonstrate empathy, concern, and interest in providing quality care and service to our elders.
Ability to interrelate well with others and establish positive relationships with elders, their families, other staff members, volunteers, and the public.
Ability to adapt quickly in a progressive change-oriented environment.
Ability to work independently and can evaluate my own progress and take responsibility to grow in my position.
Maintains an interest in learning more about person-directed care.
Ability to manage the associated physical, emotional, and psychological demands of the work involved.
General understanding of computers and associated programs required in the everyday work of the LPN.
