Student Information Form

This form helps us understand how you learn best, what challenges you may experience in academic settings, and what supports could help you be successful.
You are not expected to have all the answers—your Student Success Advisor will work with you to identify appropriate supports and accommodations.

You can register with Accessibility Services without a diagnosis. An advisor will work with you to explore your needs and guide you through any documentation requirements.
