Clinical Supervisor - LPN

Qualifications

REPORTING RELATIONSHIPS

 The Supervisor reports directly to the Senior Manager of Operations.   

 SUMMARY OF POSITION:

 Accountable for supervision and clinical support of HCA’s and shift nurses along with the Nursing Care Centre functions and staff. Liaising with clients and their families to ensure quality care. Manages 3rd party funder relationships

Responsibilities include:

 Health Care Aides: Interviewing and onboarding HCA’s for private care clients and third party contracts, initial assessment of private care clients, creation of client care plan, reassessment of care plan as needed, performance management and education and support.

 Shift Nursing: Interviewing and orientation of shift nurses for private care and third party contracts, education and training as required, coaching, performance management and monitoring of client care.

 Nursing Care Center: Supervision of nurses at the clinic site in Hamilton, oversight of clinic operation, metrics and reporting. Coaching and performance management of clinic staff.

 Manger on-call Rotation: The supervisor will participate in after hours coverage supporting the on call Nurse.

 SCOPE OF PRACTICE:

 The Nursing Supervisor must be a licensed LPN practicing within the Standards of the CLPNM. The Supervisor may be required to provide direct care as needed.

·         Consistently meet KPIs related to role/field staff service delivery
·         Positive customer service
·         Management of client/family/funder complaints or concerns

Leading Care Providers
·         Successful staff retention
·         Leading by example; promoting service delivery excellence
·         Respond to the identified staff training needs with action plans to resolve gaps,

minimize risks and ensure safe service delivery
·         Provides professional expertise related to clinical treatment and client services

Reporting
·         Ensure all reporting meet or exceeds all standards and expectations
·         Identifies opportunities to improve service
·         Ensure documentation is completed accurately and on time

Quality and Risk Management
·         Actively identify risk and work with stakeholders to mitigate or manage risk
·         Ensure a timely and comprehensive response to incident reporting
·         Actively participate and at times lead quality based initiatives
·         Facilitates effective communication strategies to ensure quality care 

QUALIFICATIONS/REQUIREMENTS:

Graduate of an approved nursing program
Current registration in good standing with the CLPNM
Minimum 3 years’ experience in community health care
Experience in health care supervision or leadership preferred
Valid Manitoba drivers license, reliable vehicle. Travel is required.
