Program Overview:

St.Amant Respite and Family Support Services offers out of home respite options for families with children who have significant disability-related care needs and who do not have the necessary resources to implement a self-managed respite plan and who may require a longer break than other supports can provide.

The 24 hour shift staffed home provides planned and regular out of home respite for children & youth ages 10 – 17 years who are not in care and are living with intellectual or developmental disabilities and/or Autism. This comprehensive service also provides enhanced support to families and their children which may include occupational therapy, behavioral supports training and counselling, with the goal of strengthening families’ abilities to continue to parent their children at home.

Position Summary:

The Team Leader is responsible for the overall support and service provided to people within assigned community home(s). The Team Leader manages residential services in accordance with the Mission and Values of the organization and provincial legislation and regulations. The Team Leader ensures that services are provided in a ways that optimizes dignity, self-determination, and attainment of personal outcomes.

The Team Leader supervises Direct Support Professionals within the assigned homes. The Team Leader develops and supports good team communication and collaborative working relationships by spending time in homes, making observations and providing feedback. Team Leaders assess the scope of each team members’ potential contribution and assist each team member to achieve their full contribution. The Team Leader may be required to provide direct support in the case of urgent or emergency situations.

Qualifications

Qualifications:

Educational:

• Post-secondary certificate, diploma or degree in a relevant social science or human services field is preferred
• Certification in CPR and Emergency First Aid
• Certification in Nonviolent Crisis Intervention is an asset
• Formal training in Personal Outcome Measures is an asset

Professional:

• Minimum of two years experience providing services to people with disabilities in a community setting
• Previous experience as a supervisor in an interdisciplinary environment
• Previous experience in a unionized environment
• Knowledge of care, education and training of persons living with a developmental disability
• Knowledgeable of the history of services to people with disabilities and current trends, philosophies and practices concerning their care, education and lifestyles
• Knowledge of the structure, regulations, and legislation of Family Services, Child and Family Services, and Residential Care Licensing is an asset
• Knowledge of theory of Social Role Valorization and Personal Outcome Measures is an asset
• Previous experience managing a budget within a non-profit setting is an asset
• Must understand and support the concept of interdisciplinary teamwork
• Ability to plan and implement individual support plans

• Possesses the emotional stability necessary to serve as a role model regarding cooperative teamwork, conflict resolution and process improvement

• Ability to anticipate issues/concerns and develop proactive strategies to minimize negative impacts
• Good computer skills using Microsoft Office
• Ability to lead and train staff
• Ability to participate fully in the physical care of people according to best practice (e.g. lifts and transfers and crisis management)
• Must be able to be on-call during selected after hours
• Ability to work flexible and extended hours when necessary
• Valid Manitoba Driver’s License in good standing (Class 5 – full, no restrictions). Access to use of reliable vehicle for work

Duties and Responsibilities:

• Provides direction to Direct Support Professionals within assigned homes to ensure services provided are of the highest quality and are directly related to the individual’s assessment of their personal outcomes
• Establishes and maintains effective working relationships with people supported and their families, volunteers, St.Amant staff and external collaterals
• Ensures each person’s personal outcomes, needs and preferences are reviewed by an interdisciplinary team regularly, and the process and outcomes are documented and shared with all team members
• Ensures that the people we support are supported in accordance within the principles of the Vulnerable Persons Act, reflecting the right of all people to make their own decisions and have help, where necessary, in a manner that respects their independence, privacy and dignity
• Responsible for chairing person-centered planning meetings
• Assists Coordinator to ensure coordination and integration of services
• Provides recommendations to the Coordinator regarding the appropriate complement of staff, works closely with Staff Scheduling Services to ensure adequate levels of staffing for each assigned home
• Collaborates with Family Services and/or Child and Family Services to ensure person’s needs are identified and addressed, and is responsible for following up on needs, steps and goals of each person
• Provides direction and leadership to Direct Support Professionals in crisis situations. Identifies risks, assesses and intervenes as necessary to manage crisis situations
• Promotes awareness of people’s rights and responsibilities in the least restrictive and intrusive support model
• Assists people as necessary with activities of daily living
• Articulates and presents required information regarding support, both in writing and verbally
• Monitors and assesses health and provides or accesses appropriate health care for people’s illness, injury, or trauma
• Ensures that people supported and staff live and work in a safe nurturing environment
• Ensures that personal health and other confidential information is protected
• Performs various administrative duties within the home to ensure the smooth flow of services and information in a timely and effective manner
• Guides and models individuals in areas of socially acceptable behavior that will ensure individuals are perceived positively by their community
• Provides support and services in a manner which is holistic and person-centred and community-focused
• Facilitates, mediates, and maximizes family and natural relationship involvement
• Recognizes and supports the culture and diversity of each person, and facilitates cultural celebrations and traditions
• Chairs meetings with Direct Support Professionals
• Schedules and attends appointments/meetings related to peoples’ needs
• Responsible for updating and maintaining relevant documentation related to supports, services for people supported and documentation related to team of Direct Support Professionals

As a condition of employment, successful candidates will be required to provide a satisfactory Criminal Record Check with Vulnerable Sector Screening, Adult Abuse Registry Check, and a Child Abuse Registry Check.

As a Condition of Employment, new hires must complete a series of Occupational Health checks. This includes the requirement to confirm immunization or immunity of specific communicable diseases.
