Overview

Summary
Under the direction of the Health Director, the Psychiatric Nurse - Case Manager will plan, organize, implement, direct, supervise, coordinate and evaluate activities related to the Families with Children with Complex Needs Project referred to the Jordan’s Principle CFI program.

The purpose is to provide program supports to families with children living with special needs and to help enhance the child’s life and facilitate health care interventions and developmental stimulation without denial, delay or disruption. Work with stakeholders to provide support, address gaps in service, avoid jurisdictional disputes and improve needed care.

The Psychiatric Nurse - Case Manager, will ensure the well-being of clients into the program and promotes their development. A key component of this job is to assist in their physical, social, emotional and daily life skills development based on the seven domains of development. This increases their independence and allows them to function appropriately in the community.

Responsibilities
Work in collaboration with the Northern and Winnipeg Regional Health Authority and Children’s Disability Services to ensure client needs are met while work is underway to create efficiencies, shared training, assessment methodology, and tools.
Provides support to families with children with complex needs. Services provided include respite care, crisis intervention, behavior management, and life skills training, whether on a one-to-one basis or in a group environment.
Plans, develops, implements and evaluates recreational, social and/or educational activities for children and their families.
The Psychiatric Nurse - Case Manager and Child Development Workers will strive to create a fun and enjoyable atmosphere in order to encourage client participation in programs.
The Psychiatric Nurse - Case Manager will ensure a model of service delivery for children with complex needs and their families.
Ensure systems are in place to support the delivery of the model of service.
Taking the lead in securing effective development, delivery and review of services.
Ensuring workforce requirements, training and development needs, funding arrangements and outcome/performance measures.
Developing a strategy that ensures all children are fulfilling their potential and that action must be taken to improve the lives of these children and their families who need help the most.
Recognizing the deficits within service systems and the need to protect children through early interventions, sharing of information, effective co-ordination of services and appropriate training of staff.
Uses H.E.A.T. method for dissatisfied clients – Hears them out: understands what they are upset about to accurately solve the issue; Empathizes: does not need to agree with client but is able to understand their feelings of frustration; Apologizes: they are sincere in their apology; Takes action: once the issue is clear, they take responsibility and find a solution by taking action.
Provides formal presentations and education programs as well as informal teaching to communities, families, groups and individuals.
Emphasizes health promotion and illness and injury prevention and provides appropriate anticipatory guidance.
Incorporates knowledge of behavioral sciences with teaching and learning principles when carrying out educational activities and uses strategies appropriate to the relevant target group.
Uses clinical skills to assess the client’s health status to enable joint planning, implementation and evaluation of appropriate interventions.
Uses appropriate health promotion, illness and injury prevention techniques.
Provides information and navigation for families.
Promotes and supports patient safety by addressing client safety issues and concerns as they arise.
Monitor and evaluate all tasks of the Child Development Workers and the program;
Identify persons requiring client needs;
Make recommendations for changes and improvements to the program, as required;
Schedule work assignments;
Coordinate appropriate care and equipment including other community resources, as required;
Establish and maintain a current, accurate, confidential client reporting system;
Encourage clients and families to be involved in care, if appropriate;
Liaise with all family, medical, and other resources, as required.
A model of service delivery for children with complex needs and their families;
Systems that are required to support the delivery of the model of services;
Families identified through a screening (assessment) process will receive in-home visits which will focus on parent-child attachment, challenges facing the family, the family’s strengths and needs, current connection to community resources, and personal and professional support.
Documentation of all case management activities.

Qualifications

Qualifications
Completion of an accredited Psychiatric Nursing education program
Strong verbal and written communication skills; ability to engage in meaningful and solution-focused discussions with stakeholders and partners is imperative
The ability to organize one’s self and remain organized during times of priority shifting and pressure is imperative
Strong computer and technology skills are required due to documentation and care planning requirements.
Strong negotiation, and mitigation skills and ability to remain professional and composed during times of adversity.
Ability to problem solve and communicate findings within appropriate time frames based on a risk assessment.
The successful candidate must be able to work in different settings (home environments and weather conditions due to travel needs)
