Treatment Foster Care (TFC)

Full-Time Permanent or Part-Time/Full-Time Term, Clinical Case Manager Positions

Overview: Knowles Centre is seeking Clinical Case Managers (CCMs) for their Treatment Foster Care (TFC) program. Within this role, the individual will provide direct support, consultation, training, and supervision to foster parents/licensed support providers of clients; screen prospective care providers; complete home studies/initial licensing pieces and prepare for annual license reviews; provide assessments of client’s needs; and will be responsible for the overall development, implementation, and monitoring of intervention plans for clients on their caseload.

The program is seeking applications from individuals who have either:
• case management experience with children/youth in the care of Child and Family Services to primarily oversee a caseload of foster homes, and preferably additional experience working with Community Living disABILITY Services (CLdS), to support TFC’s goal of expanding services to adults; OR
• extensive experience working with Community Living disABILITY Services (CLdS), to primarily oversee a caseload of home shares for adults living with an intellectual disability. Additional case management experience with children/youth in the care of Child and Family Services would be an asset to support transitions of youth in care to adult services.

Qualifications

The ideal candidate should have:

1) Experience working with children/youth with developmental, behavioral and emotional challenges and/or experience
working with Community Living disABILITY Services (CLdS), preferably in case managing and/or licensing home shares;
2) Experience working with children/youth/adults in CFS/CLdS, and their licensed support providers or foster/kinship families,
and birth families; 
3) Excellent assessment skills;
4) Experience and ability to develop effective treatment/intervention plans;
5) Commitment to working from a trauma-informed and strengths-based perspective;
6) Strong understanding of attachment issues and needs, and the importance of attachment-based care providing;
7) Experience working collaboratively with different systems (Child Welfare, Adult Services, Justice, Education);
8) An understanding of colonization, intergenerational trauma, Indigenous practices and traditions, and
direct experience working with Indigenous clients;
9) Excellent communication and interpersonal skills; and
10) Excellent written skills and demonstrated ability to effectively complete paperwork and meet established deadlines.

A Master’s Degree (social work, psychology, counselling) or a Bachelor of Social Work Degree and three years of experience working with clients and their caregivers is preferred. Successful applicants will have a clear Criminal Record Check, Child and Adult Abuse Registry Checks and Prior Contact Check, and possess a valid driver’s license and a reliable vehicle.
